I acknowledge that Hope For Ohana is an out-of-network provider and is not contracted with my health insurance plan. I understand:

I am responsible for all fees associated with services provided by Hope For Ohana.
I may choose to submit claims to my insurance company for potential reimbursement; however, reimbursement is not guaranteed.
Hope For Ohana may assist with documentation required for out-of-network reimbursement, but it is ultimately my responsibility to verify coverage & submit claim.

I will inform Hope For Ohana prior to beginning the assessment
